For many individuals preparing to enter trauma recovery, the prospect of facing deeply painful memories can feel completely overwhelming. Unlike traditional talk therapy, which often requires recounting every detail of a traumatic event, Eye Movement Desensitization and Reprocessing follows a highly structured, systematic protocol designed to keep you safe. Phase 1: History Taking and Treatment Planning
The first phase is a comprehensive evaluation. You and your therapist will look closely at your personal background to build a tailored emdr treatment plan. You do not need to dive deep into trauma descriptions during this phase; instead, you will identify specific target memories, current day-to-day triggers, and the negative core beliefs (e.g., "I am powerless") that you want to resolve.
Phase 2: Preparation and Stabilization
Before touching a single traumatic memory, your therapist will teach you specific self-regulation tools to manage intense emotional stress. You will establish techniques like the "Safe/Calm Place" or the "Container Exercise." This phase ensures that if you experience an emotional shift during or between sessions, you have the internal stabilization skills necessary to bring your nervous system back to a state of calm.
Phase 3: Assessment
During the assessment phase, the specific target memory is isolated and activated in your mind. Your therapist will help you pinpoint the visual image that represents the worst part of the trauma, the negative belief associated with it, and where you physically feel that stress in your body. You will also identify a positive statement you would prefer to believe (e.g., "I am safe now") and rate its validity using the Validity of Cognition (VoC) scale.
Phase 4: Desensitization
This is what most people picture when they think about eye movement therapy. While focusing on the traumatic memory and its physical sensations, you will engage in sets of bilateral stimulation (such as following moving lights or hand-taps). Between each set, your therapist will ask you to blank your mind and report whatever changes, insights, or bodily sensations arise. This phase continues until your distress level drops down to a zero on the Subjective Units of Disturbance (SUD) scale.
Phase 5: Installation
Once the memory no longer triggers a panic response, the focus shifts to strengthening the positive belief you identified in Phase 3. Through further sets of bilateral stimulation, your therapist helps "install" the constructive self-belief (such as "I am in control of my life now") into your neural networks, binding it directly to the old memory until it feels completely true.
Phase 6: Body Scan
Trauma is fundamentally stored in physical tissue. In Phase 6, you will be asked to close your eyes, think of the original memory and your positive belief, and scan your physical body from head to toe. If you notice any lingering tension, tightness, or somatic discomfort, further short sets of bilateral movements are applied to process out the physical residue of the trauma.
Phase 7: Closure
Every single EMDR session ends with a formal closure protocol, ensuring you leave the clinic feeling completely grounded and physically stable. If a memory was not fully processed during that hour, your therapist will guide you through your Phase 2 relaxation techniques to safely put the memory away in an internal mental container until your next appointment.
Phase 8: Reevaluation
At the very beginning of your next scheduled session, Phase 8 takes place. You and your clinician will look back at the work done previously to evaluate your progress. Your therapist will check if the targeted memories remain neutral and assess how well your positive beliefs are holding up in the face of real-world, daily stressors. This review dictates whether you move on to a brand-new memory target or continue processing the current one.
